MONITOR
Monitoring is important because it gives you feedback on how effective your response plan is and to spot problems or blocks and to resolve them using the same process of REFRAME. Below is a normal monitoring journal layout.
COMPLETION
It's quite important that you check off the successful completion of portions of your plan so that you can gain a sense of progress. This can also alert you to things not yet done, and which may be blocked or frustrated for some reason requiring some problem-solving.

Monitoring your functional level in mood and behaviour
You can use the suds scale to do this or the VOCs scale. It gives you a way of tracking how you're doing across a period of time. If you can see that things are not improving then your plan needs to be revised.
SUDS scale – How are you doing at present?
1. The absence of any distress. Feeling calm and totally relaxed
2. Neutral feeling or just OK, not as relaxed as could be
3. Increased discomfort, unpleasant, but in control
4. Noticeable discomfort or distress, perhaps agitation, but tolerable
5. Discomfort is very uncomfortable, but I can stand it.
6. Discomfort worsens and affects my life
7. Discomfort is severe and emotional pain interferes with my life.
8. Discomfort increases and it is in my thoughts constantly
9. Discomfort is nearly intolerable
10. Discomfort is extreme and the worst imaginable. I feel panicky and overwhelmed.
The VOCs scale – How confident are you the plan is working?
0. No belief in the plan at all.
1. Able to consider the possibility of the plan.
2. Able to feel some hope that the plan could be workable
3. Able to relate to prior experience supporting the possibility of working the plan.
4. The plan has a grain of workability in it.
5. The plan is beginning to be workable.
6. Able to see that the plan is already partly workable.
7. The plan feels workable but there are some reservations or disclaimers.
8. The plan feels mostly workable but there is still some doubt..
9. The plan feels almost completely workable.
10. The plan feels completely workable - I believe it without a doubt.

Make a list of problems, they won't just fix themselves always.
