REFRAME ACTION PLAN worksheet (Problem Solving)
This form is for when you identify a problem issue you can ACT on to adjust reality. The goal is to make an action plan for solving it.
After you've actioned your plan, use your emailed copy, come back online, and fill in the appropriate section. A word processor is handy for this as you can cut and paste.
(It may help you to print off a copy of this page for reference)
1. Figure out your GOAL in specific terms not general.
2. Work out the action part in respect of bringing the goal to pass. Anchor it in terms of WHEN, WHO and WHERE.
3. Take time to identify possible problems and develop strategies to overcome them.
4. Come online and enter your complete ACTION PLAN and submit it. A copy will be sent to you if you request it.
5. EACH time you work your plan keep a record of what progress you made with it. IF things don't work out, journal using the journalling link to receive help to see what beliefs are getting in the way. You can use this form to email your results, just use the progress section but identify who it is who is sending the results.
